Abstract

Conservative therapy for gastroduodenal bleeding caused by gastric ulcer and duodenal ulcer is often ineffective. Surgical treatment remains one of the most reliable methods of stopping bleeding, however, operations at the height of bleeding are accompanied by high mortality, reaching 16-30%. In this regard, for many years, clinicians have been trying to use various local methods of influencing the source of bleeding during endoscopic examination in cases of gastrointestinal bleeding. Endoscopic therapeutic interventions are aimed at stopping ongoing bleeding and reducing the likelihood of its resumption in the near future, which makes it possible to prevent massive blood loss, and also, by reducing or eliminating hemodynamic disturbances, prepare the patient for a delayed operation, and in some patients, avoid surgical intervention altogether in conditions of acute blood loss and, if necessary, perform it on a planned basis. In clinical practice, various methods of influencing the source of bleeding through an endoscope have become widespread: diathermocoagulation, laser photocoagulation, cryotherapy, application of film-forming preparations, irrigation and injection of hemostatic solutions, administration of vasoconstrictor and sclerosing preparations, and others.
