Modified San Diego Acetabuloplasty versus Pemberton Acetabuloplasty for Patients with Developmental Dysplasia of Hip

Developmental dislocation of the hip (DDH) is a heterogeneous condition that includes a number of hip problems. It is of utmost importance to promote early detection and treatment in order to enhance the quality of life affected children with their families. There is no consensus among orthopedic surgeons about the optimal or ideal method of treatment. Several treatment approaches are currently utilized and different modification are applied in many centers. Reshaping osteotomy usually associated with high correction rates among these methods Pemberton and San Diego acetabuloplasty are commonly used. However, no previous studies that compare the outcomes of these two methods. Hence, we aimed to compare the clinical and radiographic outcomes of Pemberton and modified San Diego acetabuloplasty among 88 Iraqi children aged between 18 months and 9 years, in Karbala city who had Developmental dislocation of the hip. We assigned the patients into two groups; 40 patients with 60 hips treated with modified San Diego acetabuloplasty and 48 patients with 62 hips managed by Pemberton acetabuloplasty. Both groups compared with regard to the using clinical outcome modified McKay criteria, radiographic outcomes, radiologic outcomes using Severin’s classification and the incident complications. The study found that modified San Diego acetabuloplasty is a safe and effective alternative to treat acetabular dysplasia in patients with typical DDH. No significant clinical and radiological differences were observed between the two groups. Less bleeding, Shorter operative time and less exposure to radiation are reported with the modified San Diego osteotomy.
